Insurance 

In order to best serve you, State Avenue Dental Office is in-network with most of the major PPO dental insurance companies. Dental coverage and benefits may vary depending on the plan and network status. 

When making your first appointment, please have your dental insurance information on hand in order for our schedulers to verify your plan’s network status and benefits. Having this information before your first appointment will help optimize your time in our office. 

We are happy to verify your insurance and answer any questions you may have once you have scheduled your appointment! 

Medicare 

Medicare is accepted at our office, but there are some plans that do not have network status with us. In order for us to check your plan’s limitations, please provide us with your insurance information upon scheduling. 

Local Non-Profit

Our office has coordinated a referral program with a local non-profit. They accept referrals for our established patients who are Kansas residents and would like to apply for free dental insurance. They provide excellent benefits for our patients in need of dental assistance. Eligibility depends on qualifying factors that the non-profit specialist will explain at the time the patient is contacted. For more information on how to qualify, please speak with our front desk administrators. 

For our patients who do not have or do not qualify for any form of dental insurance, our office offers an in-office discount program. When scheduling or checking in to your first appointment, please ask for more information and we will be happy to assist you!
